Do I need a physician referral?
No. You can call (601-288-FHIN) to schedule an appointment without a referral.
Can I bring a friend or family member with me?
Yes. A trusted friend or family member is welcome, as they can be an important part of your support team. It can be helpful to let your companion know in advance what you want from your visit, and they can help remind you if needed.
How long will the appointment take?
New patient appointments are approximately 45-60 minutes. Follow-up appointments will take approximately 30-45 minutes.
How can I access my information online?
Forrest Health uses Iris, a free and secure online platform that allows you to access your health information and communicate with your provider. Click HERE to sign up or login.
What are your hours?
Monday-Thursday, 8:00am-4:30pm and Friday, 8:00am-12:00pm
Before Your Appointment
- Prepare to arrive 10-15 minutes early. This will allow time to fill out any necessary paperwork and you can gather your thoughts before meeting with your provider.
- Have a list of all medications and dosages, including supplements, you are taking.
- Have your insurance card and medical tests results related to the condition.
- Keeping a log or journal is helpful.
- Be prepared to describe your symptoms and how they impact your life. Please include any progression.
- Sometimes it is easier to bring a list of questions you may have with you rather than try to remember.
- Bring a loved one with support, to help take notes, and/or ask questions.
