Hospice Services
Hospice is a special way of caring for the terminally ill and their caregivers. Focusing on the whole person and not the disease, Hospice strives to take care of the patient's physical, emotional, social and spiritual needs, as well as the needs of the family. Hospice takes a team approach with each member of the team, offering a specific set of skills.
The special needs of a hospice patient require special attention from the hospice team. Hospice consists of an interdisciplinary team of medical professionals who possess the knowledge and ability to provide superior care in any situation.
Our professional staff create and coordinate an individualized plan of care for each patient. The patient's primary physician maintains the primary responsibility for the patient's care and is continually updated on their status by the hospice team. The team consists of:
- The patient's physician
- Registered nurse
- Nurses' assistants
- Social workers
- Pastoral counselor
- Bereavement coordinator
- Volunteer coordinator
